Welcome to personal goal setting forms part two, of your personal goal setting activity.
The goal form worksheet is divided into four categories, representing your four core health areas. Once you have identified the values
which are of most importance to you in each of the following
areas, simply write them under the appropriate headings.
During this part of your goal personal setting activity, don't
worry too much about getting your goals absolutely perfect, since we will be covering more fully, this aspect of your goals worksheet next month. Do however, keep your goal positively phrased and ensure it is about what you want to bring into your life - NOT what you want to get rid of. You can of course use these personal goal setting forms part two, to set as many goals as you want. By this time next month, you will have prioritised one or two you will definitely want to start pronto!
